凡以清解里热为主要作用的药物,叫做清热药;以清热药为主组成的方剂,叫做清热剂。

清热药和方,性属寒凉,具有清热泻火,解毒凉血等功效,主要用于热病高烧,热痢,痈肿疮毒等所呈现的各种里热证候。

热证中有热在气分、营分、血分、以及虚热、实热等证的不同,故清法中可分为清热泻火、清热凉血、清热解毒、清热燥湿药四种;别附;清虚热、清脏腑热法。

本类方药性多寒凉,易伤脾胃,对脾胃虚弱的患者,宜适当辅以健胃的药物;热病易伤津液,清热燥湿药,又性多燥,也易伤津液,对阴虚的患者,要注意辅以养阴药,祛邪不忘扶正。

对脾胃虚弱、胃纳不佳、肠滑易泻的慎用。

【组成】

  黄芩15克,黄连15克,陈皮6克,甘草6克,玄参6克,柴胡6克,桔梗6克,连翘3克,板蓝根3克,马勃3克,牛蒡子3克,薄荷3克,僵蚕2克,升麻2克。

【主治】

  大头瘟。恶寒发热,头面红肿灼痛,目不能开,咽喉不利,舌燥口渴,舌红苔白兼黄,脉浮数有力。(本方常用于丹毒、腮腺炎、急性扁桃体炎、淋巴结炎伴淋巴管回流障碍等属风热邪毒为患者。)

【正文】

    功用:清热解毒,疏风散邪。

用法:水煎服。

禁忌:斟酌。

方解:本方主治大头瘟(原书称大头天行),乃感受风热疫毒之邪,壅于上焦,发于头面所致。风热疫毒上攻头面,气血壅滞,乃致头面红肿热痛,甚则目不能开;温毒壅滞咽喉,则咽喉红肿而痛;里热炽盛,津液被灼,则口渴;初起风热时毒侵袭肌表,卫阳被郁,正邪相争,故恶寒发热;舌苔黄燥,脉数有力均为里热炽盛之象。疫毒宜清解,风热宜疏散,病位在上宜因势利导。疏散上焦之风热,清解上焦之疫毒,故法当解毒散邪兼施而以清热解毒为主。方中重用酒连、酒芩清热泻火,祛上焦头面热毒为君。以牛蒡子、连翘、薄荷、僵蚕辛凉疏散头面风热为臣。玄参、马勃、板蓝根有加强清热解毒之功;配甘草、桔梗以清利咽喉;陈皮理气疏壅,以散邪热郁结,共为佐药。升麻、柴胡疏散风热,并引诸药上达头面,且寓“火郁发之”之意,功兼佐使之用。诸药配伍,共收清热解毒,疏散风热之功。
